Final Play of Miami and Duke

I can't believe there isn't a forum for this yet, but I did not see this play live, and I have not seen the review. From what I understand, though, a block in the back was called on the field, but was somehow forgotten after the review when Miami was given the touchdown.

Is it even possible to pick up a flag after review? I recall two years ago, we were playing Boston College and a defensive pass interference was called on the Eagles. The officials went back and reviewed the play and overturned the pass interference because the ball was tipped at the line of scrimmage. That was even controversial, but looking back, I can somewhat understand it. How is it possible to pick up a block in the back penalty though, especially when it's as obvious as it was?
